About Us

       The Family Care Network (FCN) is a unique regional program spanning Alameda and Contra Costa Counties that brings together providers for the provision of coordinated, client and family-centered  medical and support services for women, children and youth living with HIV/AIDS. We provide care and referrals, regardless of ability to pay.

 

FCN’s model of care works by coordinating among multi-disciplinary providers for a multitude of services including:

 

¨ Adult, Youth and Children’s Primary Medical Care

¨ HIV Testing and Counseling

¨ Pregnancy Health Care and Perinatal Advocacy

¨ Clinical Case Management

¨ Mental/Emotional Health Counseling

¨ Legal Assistance, Benefits Counseling

¨ Substance Use Counseling

¨ Peer Advocacy and Practical Support

¨ Information and Referrals to Other Services

¨ Health Education and Treatment Advocacy

 

       The Family Care Network has established one-stop services, offered where the client receives medical care. Providers coordinate their schedules so that a client may receive legal and benefits counseling, psychotherapy and/or peer support during a visit for medical care. Some of the Network’s agencies have multiple work sites and will meet clients at location convenient to them, or schedule home visits upon request.
